UTI in Pregnancy

Send MSU for culture and start antibiotics. 

Short-term use of nitrofurantoin in pregnancy is unlikely to cause problems to the foetus. Avoid in renal impairment.

Avoid trimethoprim if low folate status or on folate antagonist (eg antiepileptic or proguanil).

First linenitrofurantoin

(avoid in third trimester)

100mg m/r twice daily

7 days

Second linetrimethoprim

Avoid in 1st trimester

200mg twice daily

7 days

Third line: cefalexin (can be used in any stage of pregnancy)

500mg twice daily
