Antibiotic Prophylaxis in Surgery - Drug Dosing

 ANTIBIOTIC DOSE ADMINISTRATION PROLONGED SURGERY - RECOMMENDED REDOSING >1.5L BLOOD LOSS - REDOSE AFTER FLUID REPLACEMENT
Amoxicillin 1g IV Bolus over 3-4 minutes  500mg after 4 hours  500mg
Cefuroxime 1.5g IV Bolus over 3-4 minutes  750mg after 4 hours  750mg 
Ciprofloxacin 1000mg PO 1 hour pre-procedure  N/A  N/A
Co-amoxiclav 1.2g IV Bolus over 3-4 minutes  600mg after 4 hours  600mg
Clindamycin 600mg IV Infuse over 10-60 minutes  300mg after 4 hours  300mg
Flucloxacillin  1g IV Bolus over 3-4 minutes  500mg after 4 hours  500mg
Gentamicin  

3mg/kg IV

Use ideal body weight (IBW) if >20% overweight - see below

 

Do not repeat dose unless >1.5l blood loss

 

Bolus over 2-3 minutes. Can also be given as short infusion

 

If surgery >8 hours, re - dose at half pre-op dose if no renal impairment
