Respiratory Tract Infections

First Line

Amoxicillin 500mg tds PO

Escalation

Co-amoxiclav 1.2g IV 8hrly + Erythromycin (if atypical pneumonia is suspected/likely or confirmed) 500mg qds PO

De-escalate IV Co-amoxiclav to

Co-amoxiclav 625mg tds PO+/-  Erythromycin 500mg qds PO when suitable

Penicillin Allergy

Erythromycin 500mg qds PO

Escalation

Ceftriaxone 2g IV daily + Erythromycin (if atypical pneumonia is suspected/likely or confirmed) 500mg qds PO

Notes

Erythromycin may cause GI upset in breastfed infants

Ensure sputum and atypical pneumonia screen (if appropriate) are sent ASAP

During the influenza season ensure viral throat swabs/gargles are sent and consider using antivirals if clinically indicated
